Jean Culver, MD, MPH, Aviation Medical Examiner, is certified to perform second and third class FAA physican examinations.

Pilots now have the option of completing the history section of the certification physical examination online by going to the FAA web site. After completing the history, the pilot should print a copy of the 8500-8 form, to be brought to our clinic the day of the physical along with a picture ID and confirmation number.
